There is a little bit of confusion on the web regarding the parental drivers education affidavit. In some sources, the affidavit is labeled DL-90A, in others - DL-90B. Both of these labels are correct as the parental driver education affidavit is actually a generic name for two forms - Classroom Instruction-Driver Education Affidavit - DL-90A and Behind the Wheel-Driver Education Affidavit - DL-90B.
Parent Taught Drivers Ed In Texas
Both the DL-90A and DL-90B are part of the paperwork required for Texas teenagers to be able to go through the Texas parent taught drivers education course. One of the main conditions of the parent taught drivers education classes in Texas is prior pre-approval of the instructing parent by the DPS. The parent submits the application for parent taught drivers education package, DL-92, to the DPS and if the DPS grants its approval, they issue the DPS parent taught drivers education package that contains both the DL-90A and DL-90B.
Classroom Instruction Driver Education Affidavit - DL-90A
The classroom instruction drivers education affidavit is the paper that grants the parent legal right to provide drivers education lessons to their child. Drivers ed lessons can be provided according to the instructions from the DPS parent taught drivers education package or according to one of the Texas drivers education classes offered by commercial schools.
Behind The Wheel Drivers Education Affidavit - DL-90B
The behind the wheel drivers education affidavit is the paper that grants the parent legal right to conduct in-car driving lessons to their child. Behind the wheel lessons can be provided according to the instructions from the DPS package or according to one of the commercial drivers ed courses.
DL-90A and DL-90B Replacement
At the moment, you cannot download DL-90A or DL-90B. If the form was misplaced, you should contact the DPS for a replacement.
Both of these forms must be provided at the time when the teen applies for the Texas learners permit and Texas drivers license, along with the following documents:
- Application for Texas drivers license, DL-14A
- Proof of identity
- Proof of social security number
- Verification of enrollment and attendance form in school
- The learners permit/drivers license fees

DL-90B (Rev. 9/13)
The reason why you cannot download the drivers education affidavit off the Internet is quite simple – the DPS does not allow for that form to be publicly released and driving schools will never go against this rule. The forms you may have downloaded online are hopelessly outdated. The current version of the form was revised in September of 2013 and it will have a Rev. 9/13 stamp on it, while most the forms you can find online are from 1999. If you have already downloaded a copy of the affidavit, take a look at the bottom of the page. Here is what the current affidavit looks like.
Note how the bottom left corner has DL-90A (Rev. 9/13) printed on it. If your paper says Rev. 9/99 (or anything else, for that matter) – chuck it away and don't even think about taking it into the local DPS office, it will not be accepted and you will just waste your time on getting to the office and waiting in the DPS lines. When it comes to DL-90A and DL-90B, you must use the forms that came with the parent taught drivers education packet, nothing else will do!
