Form DL 14A can be obtained at any drivers license office or you can also download the application from the DPS website.
How Do I Fill In DL-14A
You can fill in the application for Texas drivers license at home or at the DPS office when you get there. Please note that all the information on the application must be typewritten or printed in back ink. You should only sign DL-14A in the presence of a notary public or a DPS official, otherwise your signature will not be accepted. You will also be required to provide your social security number on the form.
Parental Authorization
If you are under 18 years of age, your parent or legal guardian will need to be present at the time the application is signed. The legal guardian will need to sign the Parental Authorization part of the form to affirm their consent with your application.
