When you apply for the Kentucky drivers license or and ID card, you will be required to fill in the Kentucky Drivers License/ID Card Application Form. Please note that the form must be signed in front of the DDL official, you cannot sign it at home and then bring it to the office with you.
If you are under 18 years of age, your legal guardian will also need to sign this form.
