The certificate of attendance in Georgia must be presented at the DDS when an underage student applies for the Georgia drivers license. The certificate of attendance must be signed by the student’s school officials.

You can download a sample copy of the certificate from the DDS website. You will need to print it out and take it to your high school to be signed by the school representative.
What If I Am Home Schooled
Home schooled students in Georgia are also required to present the certificate of attendance when they are applying for their driver’s license. In case the student is being home schooled, the form must be completed by the local school superintendent’s office.
